Medical Documentation
- Glasses/contact lenses (if needed to meet vision standard)
- Hearing aids (if used)
- Medication list
- Specialist clearance letters for:
  - Diabetes
  - Hypertension
  - Cardiac disease
  - Seizure history
  - Sleep apnea (with CPAP compliance printout)
  - Anticoagulation therapy (INR reports)
Prior Health Records if applicable
